Some more updates. I found a problem with the footer for the theme missing the closing </body> and </html> lines. This has been an issue that has something to do with another plugin stripping them out of the footer. I had this problem with another site and that is how I found this problem. I had started the whole delete and re-install process with WP-Super-Cache to see if I could make it work this time. That is how I found and fixed this problem, so now the HTML code is being generated properly.
Again there is the issue of Server errors however. This code seems to be generated when I select the 'Use mod_rewrite' option.
