I have read many many support tickets and tried nearly everything they suggested in order to fix this problem and append the beloved two lines

<!-- Cached page generated by WP-Super-Cache on xxx -->

I really do not know where to start in order to help you. I have I am currently using the mod_rewrite caching option.

Can somebody please help me debug this problem?
